JOB SUMMARY:

Under general supervision, plans, directs, and evaluates total nursing care and function in one unit on a particular shift, in accordance with established philosophy. Responsible for orientation, training, and management of personnel. Relates effectively with other shifts for continuity of care; participates in all phases of education, maintenance of records, and upgrading of policies, procedures, and skills of personnel.

ESSENTIAL FUNCTIONS:

JOB SUMMARY:

Works under the direction of a qualified professional nurse.  Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of his or her preparation.  Cooperates with the members of the nursing and medical staffs and other hospital departments.

ESSENTIAL FUNCTIONS:

1. Provides direct and individualized nursing care to an assigned group of patients based on the application of scientific principles, under the supervision of a registered professional nurse.
2. Assists in preoperative, postoperative, and relevant health teaching for patient and family.
3. Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care.
4. Participates in team conferences, sharing knowledge, information, and ideas among team members.
5. Prepares, administers, and records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse.
6. Performs more‑complex nursing activities as competence increases.
7. Participates in emergency care.
8. Communicates frequently with the team leader regarding patients’ physical and mental conditions.
9. Records information concisely, accurately, and completely.
10. Identifies and utilizes appropriate channels of communication.
11. Constructively participates in appropriate committees within the nursing department and/or the hospital.

ADDITIONAL RESPONSIBILITIES:

1. Floats as requested by the team leader and/or supervisor/staffing coordinator.
2. Assists in the orientation of new personnel when requested.
3. Performs other related duties as assigned.

JOB SUMMARY:

The nursing assistant performs nursing procedures and patient care tasks.

ESSENTIAL FUNCTIONS:

1. Provides for patient’s personal hygiene gives bedpans, urinals, baths, backrubs, shampoos, and shaves, and assists with showers.
2. Provides for activities of daily living assists with meals and feeds patients; ambulates, turns, and positions patients; provides fresh water and nourishment between meals.
3. Administers enemas, douches, non-sterile dressings, surgical preps, ice packs, heat treatments, sitz and therapeutic baths, and applies restraints.
4. Performs I &O; checks vital signs and weight; performs sugar and acetone urine testing, specimen collection, and post‑mortem care.
5. Provides for comfort needs of patients.
6. Works effectively as a team member in the delivery of care and assists in the orientation of new employees.
7. Utilizes resources and materials in an efficient and safe manner.
8. Maintains a safe and attractive environment for patients and co‑workers, transports patients and equipment as appropriate.
9. Promptly answers patients’ call lights and requests.
10. Reports observations of a patient to the nurse caring for the patient to ensure professional assessment.
11. Instills confidence in the patient by maintaining a neat and attractive appearance and performing duties in a competent and caring manner.
12. Shows dependability by being punctual; maintains consistent attendance.
13. Maintains good working relations among all nursing unit personnel and other hospital employees through appropriate communication.

ADDITIONAL RESPONSIBILITIES:

1. In special areas, performs duties as defined on specialized‑skill list.
2. With specialized training, and written approval, may perform additional and specialized tasks pertinent to assigned area.
3. Accepts reassignment as needed to provide necessary care for all patients.

EDUCATION AND EXPERIENCE:

1. High school graduate preferred.
2. Successful completion of nursing assistant course or the equivalent.
3. Previous hospital experience with acceptable references preferred.
4. Must pass the pre‑employment test for nursing assistant.

ESSENTIAL FUNCTIONS:

Provide one-on-one companionship and assistance to assigned patients who need constant observation.  They may be “at-risk” patients (altered mental status, suicidal, combative, etc.)

1.   Report to assigned patient on time and appropriately groomed.
2. The PSC will be responsible for notifying nursing personnel for exceptions from the patient’s established plan of care.
3. Monitor patient safety and monitor any change in patient condition.  Assure that equipment such as life-alerts, call lights, ambulatory monitors (ankle alerts) are intact or in reach.
4. Remain at the facility with the patient for the assigned shift and does not leave the patient alone at any time.
5. Assist Patient to toilet, dressing, walking and keep Patient always clean!!!! Assist Patient with daily live skills!!!

EXPECTATIONS

1.   The PSC should not recline in the recliner while at the patient’s bedside.  Use of pillows and blankets are also prohibited. Sleeping is NEVER allowed.
2. You must always give your full attention to the patient.  This means you should not be reading, texting, doing homework or on the phone.
3. PSC should get a brief report from the nurse caring for the patient at the beginning of the shift and report to the nurse at the end of the shift, as well as give report to the person next on shift.
4. Accompany the patient where he/she may go within the facility. Never leave the unit or facility with a patient to go outside.
5. Never discuss patient care with anyone (including family).  If the physician mistakes you for a family member or nurse, please inform the person that you are a safety companion, and they would need to speak to the primary nurse.
6. If there is an emergency, inform the agency immediately.
7. Caregivers are not allowed to have visitors for any reason nor are personal items (including food or drink) allowed in a patient’s room.
8. All interpersonal relationships reflect respect, concern and courtesy for the individual patient, family members, and/or significant others.
9. If there are consistent problems with your job performance, you will be asked not to return.

ADDITIONAL RESPONSIBILITIES:

Caregiver may be asked by the facility to answer questions from family members regarding client’s condition.

Communicates respectfully and clearly to client and their family members.

Other “hands off” responsibilities may be required as needed.

QUALIFICATIONS:

1. Must possess a cheerful, “upbeat” disposition which will provide positive atmosphere for the client.

• Demonstrates sound, reasonable judgment toward their responsibility of the assigned client.
• Patience and empathetic manner in dealing with patients who may be disagreeable or demanding.

Must have a minimum of one year experience as a caregiver.

JOB SUMMARY
Primary care nurse practitioners are generalists who work in family health clinics or hospitals.
They provide general and preventative care, conduct check-ups, treat illnesses, order lab tests
and prescribe medication for children and adults.

A nurse practitioner is an advanced practice nurse that helps with all aspects of patient care,
including diagnosis, treatments and consultations. They may work in both inpatient and
outpatient situations and can perform independently or as part of a treatment team. Generally,
nurse practitioners perform the important task of educating patients about preventative care and
prescribed treatments. They may also conduct physicals, order tests and serve as a patient’s
primary healthcare provider. Some nurse practitioners are also able to prescribe medications.

DUTIES AND RESPONSIBILITIES
Although there are common duties and responsibilities of all nurse practitioners, they generally
vary by specialty. Specialties include primary care, pediatrics, geriatrics, oncology and
psychiatric care.

ROLE
The Nurse Practitioner is a licensed independent practitioner who is responsible for managing
health problems and coordinating health care for the perioperative patient in accordance with
State and Federal rules and regulations and the nursing standards of care (assessment of health
status, diagnosis, development of plan of care and treatment, implementation of treatment plan,
and evaluation of patient status) 1, 2. Perioperative preparation and postoperative clinical
management is conducted in collaboration with other health care team members.

ESSENTIAL RESPONSIBILITIES

1.  Functions independently to perform age-appropriate history and physical for complex
   acute, critical, and chronically ill perioperative patients.
2.  Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific
   needs.
3. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
4. Implements interventions to support the patient to regain or maintain physiologic
   stability, including but not limited to serving in the first assisting role.
5. Assists with the provision of surgical care in accordance with facility, state, and federal
   regulations.
6. Monitors the effectiveness of interventions.
7. Facilitates the patient’s transition within and between health care settings, e.g. admitting,
   transferring, and discharging patients.
8. Collaborates with multidisciplinary team members by making appropriate referrals.
9. Facilitates staff, patient and family decision making by providing educational tools.

MINIMUM QUALIFICATIONS

• Graduate of an accredited school of nursing
• Graduate of an accredited Nurse Practitioner Program
• Current Advanced Practice Registered Nurse (APRN) Licensure
• Current Nurse Practitioner certification in specialty area
• Current CNOR/CRNFA (if applicable) certification
• Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support
  (ACLS); PALS provider card as required by department and facility
• Strong computer skills, including Word, Excel, and PowerPoint
• Eligible for hospital privileges

EXPERIENCE
Minimum of 1-2 years OR clinical perioperative experience. 1-2 years’ experience in the Nurse
Practitioner role preferred. Recent graduates are also encouraged to apply.
